I-PaCS: Integrated Primary Care and Community Support Toolkit
I-PaCS integrates community health workers in a population health model that has been shown to improve health outcomes for people with complex social and medical needs. It has also reduced people’s health care costs in New Mexico in a majority-minority community. This toolkit includes a full report on the model and guidance on how to implement it in your own community.

This report from Visión y Compromiso focuses on the workforce priorities needed for the community transformation model. It also reflects on the major systemic challenges and opportunities facing promotores and CHWs who work under this model. The report includes recommendations to advance this model at the state and national levels and ensure its long-term sustainability.
